A quick reference for most service needs
Enrollment and Change Application
Use this form to enroll new employees for health and life coverage, as well as to make changes to an existing employee’s coverage (add dependents, including newborns, change life beneficiary, change address or telephone number)
Plan 1, 2, 3 or 4 Medicare Prescription Drug Notice
Give to all employees enrolling in Plan 1, 2, 3 or 4 to notify them that the prescription drug coverage constitutes ‘creditable coverage’ as defined by Medicare Part D
Effective 10/1/2009, Plan 10 coverage is no longer 'creditable' for Medicare Part D
Plan 10 & 20 Medicare Prescription Drug Notice
Give to all employees enrolling in Plan 10 or 20 to notify them that the prescription drug coverage does not constitute ‘creditable coverage’ as defined by Medicare Part D
Effective 10/1/2009, Plan 10 is no longer 'creditable' for Medicare Part D
Declination of Coverage
Completed by all new eligible employees who are rejecting health coverage (a valid reason to reject coverage is having other group coverage, most often through a spouse – having no coverage or individual coverage may cause the firm to not meet the participation requirements of the plan)
Request/Reject Continuation of Group Health Coverage
Please have terminating employees on the health plan complete upon leaving employment to indicate whether they will exercise their right to continue coverage. (Cobra firms use CobraServ)

Information for Employers and Employees from the North Carolina Dept. of Insurance regarding continuation of group health benefits
Cancellation of Coverage Form
Completed by currently enrolled employees to cancel coverage for their dependents or themselves (usually when going onto a spouse’s coverage – remember that 75% of eligible employees must be enrolled to maintain the firm’s coverage)
